Appropriate Form To Charge Ex-Tenants With Damages Left Behind?

I'm a new landlord and just getting my feet wet. I had a person renting from me and they have left as of last night. Today when I went down there it was clear there are numerous damages which will certainly be expensive for me to hire people to repair.

Could someone please help answer the following questions:

1. What form do I use to charge them for these damages?

2. Where do I get such a form?

3. Will the Landlord and Tenant Board find the ex-tenant for me and is there a charge for this service?
